Client Agreement Form
    (Zone 2)

    The following represents an agreement between Perfect Ride L.L.C. and the Parent/Guardian named below.

    The following are important policies that will be strictly enforced. Please read carefully.

    -This Agreement is made effective on the date below and will terminate on the last day of the 2021-2022 school year (based on the Broward County School Calendar)

    -If the client wishes to cancel service prior to the last day of the school year a written notice must be provided 5 days in advance.

    -Services are to be prepaid, all payments will be automatically withdrawn via PayPal. 

    -If payment is not received by Tuesday, there will be an automatic late fee of $25.00 starting Wednesday

    -If an automatic payment is rejected a fee may apply

    -Perfect Ride reserves the right to suspend or cancel service at any time due to non-payment

    -Rates are not prorated. This means that even if a child only rides one day of the week, the full weekly rate is still due.

    -Perfect Ride reserves the right to cancel service due to unacceptable child behavior. Examples of such behavior are bullying, cursing, not wearing seat belts, walking while the van is operating, vandalism, and destruction of property.

    -If a child does not travel for more than two full weeks, Perfect Ride reserves the right to cancel service in order to accommodate for active customers.

    -Parents/Guardians must call or text prior to the start of the route if a child will not ride for any reason (i.e. ill, absent, picked up early) (by 5:45 AM for morning pick up or before 1:00 PM for afternoon pick up)

    -If we are not notified in a timely manner of a child's absence a fee may apply, determined by the provider

    -All children must be present in the designated waiting area.

    -If a child intentionally damages or destroys property due to their behavior, the parent will be responsible for the property damage, and service may be cancelled.

    -Time is crucial during hours of operation, Perfect Ride will not be able to wait more than 2 minutes once we arrive to the designated area. If the driver must leave, it will be the responsibility of the parent to accommodate the child’s transportation.

    -Perfect Ride operates based on the Broward County public school calendar. If classes are canceled for any reason, Perfect Ride will not operate and is not obligated to provide private transportation.

    -Perfect Ride does not provide transportation if a child is attending before school or after-school activities such as sports, band, ROTC, etc.

    -Changes of drop-off and/or pick-up locations are not allowed. In the event of a permanent address change, Perfect Ride will re-evaluate the possibility of providing service and rate is subject to change.

    All Parents/Guardians MUST agree to each and every term of the agreement in order to be eligible for service. 

    I have agreed to submit this agreement by electronic means. By signing this application electronically, I certify that I have read and understand the terms of the agreement. I also understand that an electronic signature has the same legal effect and can be enforced in the same way as a written signature.
